North Dartmoor Area Wellbeing

North Dartmoor Wellbeing provides social prescribing for individuals living in Okehampton, Black Torrington, Chagford, Cheriton Bishop and Moretonhampstead. The service is open to any adult who is socially isolated, is a carer or has 2 or more long-term conditions.

Our social prescribing service supports you in improving your health and wellbeing by helping you identify what matters most to you, then connecting you to local community services and activities which can best help you achieve these goals.

We also provide short-term support to people in Okehampton, Chagford, Cheriton Bishop and Moretonhampstead that are settling back into living at home after being discharged from hospital. Our service connects referral’s with local organisations suited to helping them in the aim of enhancing their health and wellbeing at home post-discharge and reducing readmissions to hospital.

Referrals for both these services are made by health and social care professionals, as well as voluntary and community sector groups who have been approved to make referrals by West Devon CVS. If your group would like to be able to make referrals please contact karen@westdevoncvs.org.uk.
